Diabetic Care in the Nursing Home

This webinar addresses the prevalence of diabetes in nursing home residents and introduces the challenges of co-morbidity, functional debility, prognosis, and risks of mortality associated with tight glucose control in this population. The concept of individualized diabetes management will be introduced to include the resident and family insetting goals for diabetes management. Secondly, this activity will provide a consensus on appropriate targets for hemoglobin A1C to prevent symptomatic diabetes such as acutemetabolic complications and infections while minimizing hypoglycemia. This webinar will conclude with a discussion on facility policies and procedures established by the medical director, and relevant quality assurance, performance improvement practices appropriate fordiabetes management in the nursing home.

Learning Objectives:

  • Introduce individualized diabetes management that includes resident and family goals, resident functional status, and prognosis.
  • Identify appropriate targets for diabetes care including glycosylated hemoglobin values, prevention of acute metabolic complications and infections associated with uncontrolled diabetes.
  • Identify clinical risk factors for hypoglycemia including nutritional deficiencies, mood disorders, cognitive impairment, and renal insufficiency.
