This session will review the positive outcomes of an effective advance care planning program across the long-term care continuum that leverages innovative and nationally recognized tools and resources. Participants will be able to employ new practical skills to improve their end-of-life discussions and documentation on Physician Orders for Life Sustaining Treatment (POLST) orders in the short-term, and apply the multidimensional approach needed to ensure effectiveness and sustainability of a POLST paradigm program in the PALTC setting.
