The session will discuss current demographics of this rising risk population and its impact on re-hospitalizations. It will also present how an integrated care model will foster partnership and innovative connections to providers in assisted living facilities, continuing-care retirement communities, hospital and home settings. Quality measures and care transitions will be reviewed within the context of the Centers for Medicare & Medicaid Services (CMS) Health Challenge Innovations Grant and how they align with the CMS Triple Aim.
