This session will present effective individual and interdisciplinary team approaches to providing end-of-life care for diverse populations. The role and functions of the interdisciplinary team in facilitating effective communication, key principlesand virtues, a systematic approach in caring for diverse populations, and a tool in helping communicating with patients and families with low health literacy will be presented. The S-P-I-K-E-S strategy for communicating respectfully in patient and family meetings will be presented, and case studies will be reviewed applying best practices in ethical, cultural, and health literacy appropriate care during these most vulnerable times in their lives.
