Chronic conditions such as diabetes, chronic obstructive pulmonary disease (COPD), and heart failure are prevalent in the PALTC setting. This workshop will discuss the recently updated clinical practice guidelines (CPGs) developed to improve the PALTC management of these conditions. The workshop will focus on successful strategies for implementation of the CPGs, prevention of emergency room transfers and avoidable hospitalizations, and the use of selected performance measurement indicators designed for this care setting.
