Improving Care Transitions: A Health Systems Approach

This session highlights four unique programs designed to improve the quality of care associated with transitions that emphasize system change that is both effective and sustainable. Standardized approaches involving care coordination, personal goal setting, patient order sets, and team-based patient safety conferences and information transfer will be described. The highlighted interventions are contextualized within the ever changing environment of new payment methodologies including accountable care organizations (ACOs), bundled payment, and Medicare Advantage plans.
