This program will outline the requirements and describe the integration of Medication Therapy Management (MTM) services into care planning of long-term care residents. Through collaboration, pharmacists, nurses, and physicians can improve care for residents with complex needs receiving high-risk medications, such as antipsychotics, anticoagulants, and antithrombotics. Most MTM interventions are focused on conditions such as congestive heart failure, dementia, depression, anxiety, and COPD. Residents with these complex care needs as well as their families can benefit from Comprehensive Medication Reviews and targeted follow-up interventions. Integrated collaborative practice approaches involving the resident, their nurse, pharmacist and medical director are a focus of MTM programs in long-term care.
