This session will focus on the collaborative and interdisciplinary nature of improving the quality of care and life for long-term care residents. It will feature presentations by a past AMDA Foundation/Pfizer Quality Improvement award winner on Enhancing the Implementation of a New Advance Directive Policy. In addition, there will be presentations from the winners of the 2015 Quality Improvement & Health Outcome award which are centered on programs that facilities have successfully implemented that improve the quality of life for their residents. These presentations will form the basis for discussion by Drs. Paul Katz and Daniel Swagerty. Their discussion will serve to highlight the relationship between individual Quality Improvement initiatives and larger research projects and trends that strengthen the quality of life across the post-acute/long-term care continuum.
