We thank Dr Basel for his thoughtful comments on our editorial, and we agree that telemedicine is a promising approach to both reducing unnecessary hospital transfers and the clinician workforce shortage in long-term and post-acute care, especially in rural areas. Other Center for Medicare and Medicaid Innovation (CMMI) projects are using this approach, such as the “RAVEN” project based at the University of Pittsburgh Medical Center. In addition, many private companies now offer telemedicine services to skilled nursing facilities (SNFs).

Burdensome symptoms and potentially inappropriate care practices are common at the end of life for nursing home residents. Appropriately managing symptoms and limiting aggressive care practices is key to high-quality end-of-life care. Little research is available, however, on the opinions of nursing home care providers about the impact of symptoms and practices for both residents and care facilities. Our objectives were to (1) identify common burdensome symptoms and potentially inappropriate practices at the end of life for nursing home residents, (2) develop and assess the feasibility of...

Identifying patients at risk for delirium allows prompt application of prevention, diagnostic, and treatment strategies; but is rarely done. Once delirium develops, patients are more likely to need posthospitalization skilled care. This study developed an a priori electronic prediction rule using independent risk factors identified in a National Center of Clinical Excellence meta-analysis and validated the ability to predict delirium in 2 cohorts.
