THEREFORE BE IT RESOLVED, that Post-Acute and Long-Term Care Medical Association (PALTmed) will work with stakeholder organizations and federal agencies to require the Centers for Medicare & Medicaid Services (CMS) to clearly define risk stratification indices and develop a cost-to-risk algorithm based on previous utilization data and incorporate unique patient characteristics including age and frailty to accurately measure Medicare Shared Savings Program/Physician performance and shared savings prior to further implementation of Value Based Purchasing (VBP) programs.
