BE IT RESOLVED, that Post-Acute and Long-Term Care Medical Association (PALTmed) request that the Centers for Medicare and Medicaid Services amend the Medicare Conditions of Participation for hospitals to require a medication profile (that includes indication, goals, and plans for monitoring towards goals consistent with compliance with F Tags 428 and 329) accompany the patient when there is a discharge to a nursing facility.
